Dear Parent/Guardian,
In preparation for the 2024-2025 school year, please complete the form below with your child's transportation needs for the school year. If you have more than one student in your household, please complete a form for each student.
This form must be completed no later than Friday, August 2, 2024.
ALL FORMS RECEIVED AFTER AUGUST 2ND WILL BE PROCESSED AFTER SCHOOL STARTS.
If forms are NOT received by or prior to August 2nd - STUDENTS WILL HAVE TO BE BROUGHT AND PICKED UP FROM SCHOOL THE FIRST TWO WEEKS!
It is imperative this information be completed and returned as soon as possible to assist us in completing accurate and efficient bus routes.
