The time has come to make a decision about Spring Semester. Would you like for your student to come back face to face at the middle school, receive virtual instruction at home or stay the same?

If you want your student's schedule to STAY THE SAME there is no need to do anything else!
We will need to start finalizing plans for the spring semester very soon, so the deadline for completing this form is FRIDAY December 4.
PLEASE REMEMBER - YOU ONLY NEED TO FILL OUT THE FORM IF YOU WANT TO CHANGE HOW YOUR CHILD IS RECEIVING INSTRUCTION. If you want your child to continue receiving instruction the same way they are currently - you do NOT need to do anything else. Your decision will be for the entire Spring Semester.
